ALL OFFENCE REGISTRATION

There are 2 ways to register for Dale Derkatch All Offence Hockey Clinics:

• Register Online then mail in* the signed waiver form with payment, or
• Print the Registration Form and mail in* with signed waiver form and payment to:

Upon receipt of registration, signed waiver form, and payment, you will receive a confirmation email with additional information for the clinic.  The confirmation email will also serve as receipt of funds.


Confirmation of acceptance will be made as soon as possible and will be dependent upon the number of applicants.  We reserve the right to combine sessions dependent upon the number of registrations,  in which case, ample notice will be made and reimbursement/credit options provided. 

* If you are registering within 5 days of the clinic date, you will receive an email with payment and waiver form instructions.




Please Note: Refunds will not be granted within 3 days of clinic unless accompanied by a medical note and are subject to a $20 administration fee.
